CIVILIAN PERSONNEL OFFICE OSAN AIR BASE

POSITION TITLE & GRADE: Nurse (Clinical/Care Management), KGS-0610-09 (Temporary NTE 1 year: 임시직 1년)
SALARY: Between 23205 – 33587 won per hour (Total hourly compensation when quarterly bonuses are included)
DUTY LOCATION: 51 Medical Group, Tricare Operations & Patient Administration Flight, Osan AB.
AREA OF CONSIDERATION: All current USFK employees (Priority Group 5). All registrants on the area reemployment priority list (ARPL) (Priority Group 6). Family members of US military and civilian employees of an agency of the United States whose duty station located in Korea (Priority Group 7). Outside Korean National Applicant (Priority Group 8 & 9).
NOTE: Newly hired NAF employees are required to serve no less than one year in the position to which they are initially hired before they can voluntarily accept any other position unless approved by management.
MAJOR DUTIES: Establishes and maintains positive working relationship working relationships with HNF International Centers and referral management offices. Performs inpatient rounds on all beneficiaries admitted to host nation hospitals assesses and interprets their concerns to determine specific means for assistance, which may be influenced by such factors as individual’s social situation, family ties, and other implications. Travels to HNFs regularly to meet with inpatient beneficiaries and interacts with HNF providers on behalf of the patient. Assists outpatient beneficiaries face-to-face as needed and also via telephone and email. Prioritization of assistance and liaison visits to individual host nation facilities, taking into consideration initial diagnoses, estimated duration of hospitalization, traveling distance, and other related information reported for each case is determined in coordination with team lead and/or appropriate medical professionals. Interprets during round with Chiefs of the Medical Staff and providers regarding established medical plans, surgery and required scheduled inpatient testing and diagnoses. Independently determines which actions are required to improve the patient’s physical and emotional well-being; these actions may range from informational or translating/interpreting activities, to assisting the beneficiary with identifying and obtaining support from appropriate organizations including involvement of medical or social service professional, or any combination of such actions depending on the complexity of individual needs. Ensures that any results received are obtained and translated to ensure continuity of care. Assists with discharge planning from the host nation facility assessing associated needs for logistical, medical, and administrative actions. Coordinates program services with host nation medical facilities to include providing significant outpatient support and assistance. Develops interdisciplinary care plan and other case management tools by participating in meetings; coordinating information and care requirements with other care providers, case managers, Chief of the Medical Staff (SGH), and referral management specialist. Evaluates outcomes of care with the interdisciplinary team by measuring intervention effectiveness with the team. Creates operating instruction, policies and directives related to case management and language services as needed. Maintains data files and records regarding individual patient progress, patient adherence to treatment, Primary Care Manager (PCM) initiated treatment modification. Enters these into Armed Forces Health Longitudinal technology Application (AHLTA). Helps with operational issues arising during the program. Provides patients with educational materials in the areas of depression, anxiety, and stress-related symptoms, health behaviors, maintenance of functioning and somatic symptoms such as chronic pain and chronic fatigue. Assists patients with problem solving as barriers to treatment adherence are encountered. Communicates, provides care coordination, and reinforces important treatment matters with patients in their caseload, caseload patient’s primary care provider and team, and relevant Behavioral Health (BH) specialist (external and internal setting). Communicates routinely regarding patient progress and treatment adherence via AHLTA. Communicates routinely (weekly) with the external BH specialist consultant (typically a psychiatrist or someone with medication prescribing privileges) and the embedded BH specialist (typically a social worker or psychologist with capacity to administer relevant psychosocial interventions) regarding patient status. Conveys recommendations resulting from these meeting to the referring PCM through AHLTA and other direct means. In coordination with and assisted by the Admin Support Assistant (ASA), maintains a variety of data reports through both paper and electronic methods as assigned by program staff, and submits reports as assigned by program staff with detailed and aggregate quality improvement data for the purpose of care monitoring, benchmarking, continuous enhancement and ongoing sustainment; coordinates activities with AFMOA, and higher headquarter program staff as necessary to comply with BH and PCMH program requirements; and maintains active relationship with all clinical practice staff to ensure the efficient flow of communication and records associate with the program. Participated routinely and as requested in local and national conference calls relative to program operations and reporting requirements. Performs other duties as assigned.
Other Significant Facts: Registered Nursing degree with at least 3 years of clinical experience or international clinic experience within the previous five years is required. Current Basic Life Support (BLS) certification.
MINIMUM QUALIFICATION REQUIREMENTS:
EXPERIENCE: One year of professional experience at grade level of KGS-07 or equivalent.
PROFESSIONAL EXPERIENCE: Experience which required application of the professional theories, scientific principles, and practices in the established profession of the position to be filled. Professional positions require professional, rather than technical knowledge, skills and abilities (KSAs) such as can be gained through intensive college education and training.
BASIC REQUIREMENTS: Applicants must have completed a three year/four year course of study in a ROKG authorized or recognized college or university in a field related to the job for which being considered and must possess a current nurse license issued by ROKG.
SUBSTITUTION OF EDUCATION FOR EXPERIENCE: Successful completion of two full years of progressively higher level graduate education in a field related to the position being filled is substituted for one year of professional experience required at KGS-9.
ENGLISH LANGUAGE COMPETENCY: 1. The American Language Course Placement Test (ALCPT) score of 80 or above and successfully passing the English Comprehension Interview (ECI) is required.
2. The Test of English as a Foreign Language (TOEFL) score of 540 and higher or the Test of English for International Communication (TOEIC) score of 750 and higher may be substituted for ALCPT requirement. Applicant must submit the TOEFL/TOEIC official copies of test score. TOEFL and TOEIC scores are valid for two years from the exam date.
